- Store name (ex: Ate Gigi Sari-sari Store)
- Registered mobile number sa Growsari app (ex: 0915-xxx-xxxx)
- Top up method na ginamit (ex: BPI Linked Bank Account)
- Date of top up (ex: July 1, 2022)
- Amount of top up (ex: Php 2000)
- Description of issue (ex: nag-top up ako ng 2000 noong July 1, 2022. July 3, 2022 na at hindi pa pumasok ang aking top up.)
